From PACS for radiology to VNA for the hospital

The Picture Archiving and Communication System (PACS) has become increasingly established as a central image archive for radiology since the 1990s. Now a new development for medical image data management is in the starting blocks: The so-called Vendor Neutral Archive (VNA) is a hospital-wide multimedia archive that centrally processes not only all DICOM data, but also non-DICOM data such as photos, videos, biosignals and PDF documents from the various medical departments.

VNA as a logical further development of the PACS

Existing PACS solutions offer DICOM-based archiving of medical data from the classic imaging disciplines.
More and more clinics are considering the implementation of a universal medical archive or vendor neutral archive (VNA) for the future-proof further development of their PACS. As the name suggests, these universal medical archives are completely vendor-independent, processing data in a standard format with a standard interface. Other names for this new type of enterprise imaging solution are Healthcare Content Management System or PACS-II.
A VNA has two main differences to a PACS: Firstly, in addition to DICOM image data, it also processes non-DICOM data such as videos, audio signals, PDF documents or medical device data. Secondly, it not only covers a single department such as radiology, but the entire healthcare facility. The VNA can therefore be seen as an extension of a PACS to more data types and more medical departments.

Special retention periods for image data

Anyone involved in the management and archiving of medical image data must also take legal requirements into account. For example, a retention period of ten years after the end of treatment applies to medical records. For radiation and X-ray treatments, however, the retention period under the German Radiation Protection Act (StrlSchG) is 30 years, and 10 years for X-ray examinations.

Some VNA solutions, such as the INFINITT Healthcare Platform, also manage these legal requirements efficiently with information lifecycle management. Different lifecycles can be defined for each category of information. Accordingly, the respective image data is automatically archived and deleted after a certain number of years, which significantly simplifies compliance with legal requirements.

The new Enterprise Imaging

The classic PACS software has modernized workflows in radiology and improved patient care in many respects. Nevertheless, there are limits to the system. For example, the classic PACS is generally limited to a single specialist department within the medical facility. This makes cross-departmental coordination and collaboration more difficult. For example, it is often difficult to use diagnostic data from pathology and share it in tumor boards.
Such problems and information silos are avoided with a VNA, enabling easy cross-departmental collaboration for the benefit of the patient. With a VNA, image data can be stored, retrieved and processed throughout the hospital – from any workstation. All data from the various specialist departments is available during a multidisciplinary treatment case. A VNA solution, which is sometimes also marketed as a PACS-II or Healthcare Content Management System, therefore represents a significant improvement in clinical processes.

Another challenge arises when a hospital decides to change its PACS provider.
Although PACS solutions store image data in the standardized DICOM format, migration of existing data is often not possible or only possible to a limited extent.
A universal medical archive or VNA ensures that no image data is lost when changing providers, thus increasing the medical service provider’s control over its medical data and making hospital-wide data management future-proof.
